A solid 5 day camping trip to one of our favorite spots. Felt so good to be in nature, with my phone on airplane mode, swimming in mountain lakes, letting the kids be dirty for days, with no schedule and no agenda. Was a good reminder how good it feels to really drop in ✨.
Mia was 42 when she came back to work with me. Sh Mia was 42 when she came back to work with me.

She’d already worked with me once before to conceive her first child. Back then, after two early losses, we found the real drivers no one had connected – low progesterone and PCOS. She conceived naturally and had a healthy, full term pregnancy.

Now she was ready to try for baby #2, and her OB had told her that at her age IVF was the only realistic option 🤯.

She wasn’t ready to accept that (thank goodness).

We started with the right testing - testing the right labs, at the right time, and interpreting them through the right lens. 

Next, I reviewed her diet, lifestyle and supplements and made a few intentional, targeted tweaks. 

But the biggest game changer was adding in progesterone support based on her low progesterone results post ovulation. The progesterone was the key missing piece.

Mia is now 43 and in her second trimester of a healthy pregnancy. No IVF. No major medical interventions. Just the right investigation and the right protocol built around her specific physiology.

This is what I mean when I say it’s rarely unexplained. It’s uninvestigated.

Age is real. But it is rarely the whole story. And “IVF is your only option” is not a diagnosis – it’s a default recommendation when the underlying drivers haven’t been fully explored.

Camping season has begun 🏕️ this was Owen’s first Camping season has begun 🏕️ this was Owen’s first time camping in a tent (we didn’t have the van on this journey), so sleep was hard to come by ... but the boys had a blast and the weekend was complete with a bear sighting 🐻 directly across the river from our campsite which was extra exciting 😳.
I talk about the hormone progesterone a lot. So na I talk about the hormone progesterone a lot. So naturally women ask me all the time - why is it so important to test, when should I test for it, and what’s the best method for testing?

First off, without progesterone production, your uterus wouldn’t be able to make the changes needed for a fertilized egg to become a fetus. The increase of progesterone post ovulation makes your already thickened uterine lining super sticky. Progesterone is also what stabilizes the uterine lining so the embryo can successfully implant, staying nice and cozy and secure in mama’s uterine lining. 

Without enough progesterone, it can be more difficult for an embryo to successfully implant and a healthy pregnancy to progress. 

To assess whether you are actually ovulating (confirmed by elevated progesterone levels 1 week after a LH surge), and whether your progesterone levels are staying elevated for long enough for implantation to occur successfully, I recommend getting a blood progesterone level 6-8 days after your confirmed LH surge. We want to see serum progesterone levels at least above 12 (ideally I want to see them above 15) to support successful implantation and early pregnancy. 

I also recommend testing PdG levels (a progesterone metabolite) using at-home urine tests such as @hellomiracare, @initofertility and/or @proovtest to confirm that levels remain elevated for long enough to allow for the best possible chance of successful implantation.

When testing PdG levels at home, you want to see PdG levels staying elevated on days 7, 8, 9, and 10 after a positive LH surge.

Sufficient progesterone levels during the implantation window are crucial for the health of an early pregnancy, so this time is nearly as important as ovulation! In fact, studies show that up to 75% of pregnancy losses occur due to failed implantation. Additionally, low PdG (a metabolite of progesterone) is associated with a higher rate of early miscarriage.
