In nearly 20 years of practice, I see them occur together constantly.

Your gut microbiome, vaginal microbiome, and urinary tract are not separate systems. When your gut is chronically disrupted — from antibiotics, sugar, stress — pathogenic bacteria migrate into the vaginal and urinary environment, depleting the Lactobacillus bacteria 🦠 that protect against infection, support sperm survival, and create healthy cervical mucus.

A question I get asked all the time - how can I im A question I get asked all the time - how can I improve the chances of successful implantation?!

I talk a lot about optimizing egg quality and knowing your fertile window - but doing what you can to ensure implantation is equally as important.

Implantation is the process when the fertilized egg (aka embryo) attaches to the mother’s uterine wall.

It takes the fertilized egg a few days to travel down the fallopian tubes to the uterus, so implantation doesn’t happen until about 7-10 days after ovulation.

The implantation process requires specific conditions. First, the embryo needs to be healthy, i.e. genetically normal. Second, the uterus needs to be “sticky” enough for the embryo to successfully implant. 

Sufficient progesterone levels during the implantation window are crucial for the health of an early pregnancy. Studies show up to 75% of pregnancy losses occur due to failed implantation. And low progesterone is associated with a higher rate of early miscarriage. 

The “stickiness” of your uterine lining — i.e. how receptive it will be to an embryo — depends, among other things, on estrogen and progesterone.

During the first half of your cycle, your ovaries produce estrogen, whose job it is to make your uterine lining nice and thick.

After ovulation occurs, the ovaries produce progesterone, which makes your already thickened uterine lining super sticky. 

The thickened lining is the source of nourishment for the implanted embryo during the next few weeks until the placenta develops and takes over. Without enough progesterone, it can be more difficult for an embryo to successfully implant. 

To support optimal progesterone levels: 

• Eat Pg boosting foods: grass fed butter, coconut oil, olive oil, nuts & seeds, egg yolks and wild caught seafood, pumpkin seeds, grass fed beef, oysters, leafy greens, avocado, dark chocolate, beans

• Seed cycling: from ovulation on, eat 1 tbsp sesame seeds + 1 tbsp sunflower seeds daily

• Prioritize sleep: get in bed early, black out your room, no sugar, alcohol or devices 2 hours before bed 

• Supplements: vit B6, vit C, magnesium, zinc, vitex, maca, and red raspberry leaf

I worked with a new client yesterday and during ou I worked with a new client yesterday and during our intake, she told me her story. 

She said she did everything her clinic told her to do.

Prenatal. Check.

FSH and AMH. Normal.

Thyroid. “Fine.”

But her egg retrieval came back with fewer mature eggs than expected.

Poor fertilization.

Embryos that didn’t make it.

And no explanation for why.

This happens more than it should.

Not because nothing was wrong.

Because the right things were never tested.

Before an egg retrieval, I want to know:

What is your ferritin? (Not hemoglobin — ferritin is the fertility marker)

What is your Vitamin D?

Is there any insulin resistance — even subclinical?

What does your HbA1c look like over the last 3 months?

Are your triglyceride and CRP levels elevated?

Are thyroid antibodies present, even with a normal TSH?

Is homocysteine elevated?

What is your DHEA-S?

Is your mid-luteal progesterone strong enough to confirm ovulation quality?

These markers are almost never ordered in a standard IVF workup.

Most of them are correctable in 90 days.

Which means the window before retrieval isn’t a waiting period.

It’s a preparation period. And the labs you run today can directly shape the eggs retrieved three months from now.
